(TCP)Transitional Care Plus

WHAT IS (TCP)TRANSITIONAL CARE PLUS?

The Transitional Care Plus Program provides patients with an opportunity to continue to receive restorative and rehabilitative services while in a hospital setting, “Close to Home.”

This allows patients the chance to receive Special Acute Care from other facilities and transfer to Corry Memorial Hospital, your hometown hospital, for your recovery. This may prevent a transfer to an unfamiliar healthcare organization, rehabilitation hospital or nursing home.

WHO QUALIFIES FOR TCP?

Patients who qualify for the Transitional Care Plus Program are those who meet federal criteria for skilled care. In other words, someone who has been hospitalized in the last 30 days for at least three days. These are patients who do not need the level of care that would normally be provided in an acute care hospital setting but need to regain health and strength before being able to safely return to their own homes. The following are types of patients that may qualify for our Transitional Care Plus Program:
  • Geriatric patients wanting to return to their homes.
  • Orthopedic patients such as those who have had joint or knee replacements.
  • Wound care patients
  • Patients recovering from surgery
  • Patients recovering from stroke,
  • Parkinson’s or other neurological illness
  • Patients who need IV medications
  • Patients who may need additional physical, occupational or speech therapies
  • Patients who no longer require intensive hospitalization but who still need skilled services to regain their health and strength in order to return home safely.


WHAT SERVICES ARE PROVIDED?

The Transitional Care Plus Program provides patients at Corry Memorial Hospital with:

  • Rehabilitation
  • Medication Management
  • Nutritional Consultation
  • Skilled Nursing Care
  • Discharge Planning


Important to a CMH TCP patient’s success is the development of a care plan designed to meet the patient’s specific needs. A team of professional staff meets regularly to review and revise the patients plan of care to address changing needs. This team often includes staff from Physical Therapy, Occupational Therapy, Speech Therapy, Dietary, Pharmacy, Nursing and Discharge Planning.

The Discharge Planning staff will work with the patient and his/her family to plan for discharge. If for some reason the patient is unable to meet “established” goals, the care team will work with the patient and family to determine discharge options and alternatives.

TCP INSURANCE CERTIFICATION

The Transitional Care Plus Program at Corry Memorial Hospital is certified by Medicare, the Pennsylvania Department of Health, and Blue Cross of Western Pennsylvania. With these certifications, many insurance companies will pay for this level of care.
